341 - IC27: Contemporary Management of Complex Elbow Disorders

The aim of this Instructional Course is to acquaint the attendee with the evaluation, pathoanatomy and reconstructive options for an array of complex elbow disorders. Also described will be the postoperative rehabilitation protocols and anticipated results from these interventions.
Normal elbow function is dependent on complex anatomic and biomechanical relationships. When these are compromised by trauma, pain and functional limitation result. This Instructional Course will focus on the leading causes of post-traumatic elbow dysfunction and provide strategies for their evaluation and treatment.
The Unstable Elbow: Selected pathologies that render the elbow unstable are covered. Specific discussions of the evaluation and treatment of posterolateral rotatory instability of the elbow will be conducted. The role of adjunctive treatments (ligament reconstruction and external hinged distraction) for challenging fracture-dislocations will be presented.
The Stiff Elbow: The etiologies for motion loss about the elbow are enumerated (including coverage of heterotopic ossification). The methods to address the osseous and soft tissue causes will be discussed, with emphasis on the postoperative regimens. Specifically, the routes of surgical approach to the stiff elbow will be presented in detail including: The Column Approach, Medial "Over the Top" Approach and Outerbridge-Kashiwagi (OK) Ulno-Humeral Arthroplasty. Handling of the ulnar nerve will be covered in this context. Additionally, the prevention and treatment of heterotopic ossification about the elbow will be addressed, with particular focus on the surgical excision options and postoperative regimens.
The Destroyed Elbow: This distinction is reserved for the elbow that has been severely compromised by tissue loss, articular destruction and/or gross instability. The role of salvage procedures including fascial arthroplasty, total elbow replacement and elbow arthrodesis will be defined. Also described is the role of total elbow replacement in the elderly patient with complex fracture of the distal humerus.
Role of Operative Arthroscopy: The evolving role of operative arthroscopy in the treatment of complex elbow disorders is addressed. Emphasis is placed on the indications, surgical technique, avoidance of potential complications and anticipated outcomes of surgery.
Learning Objectives:
- Familiarize attendee with logical, systematic approach to evaluation of the "destroyed elbow".
- Recognize the germinal anatomic and biomechanical concerns for handling complex elbow disorders.
- Develop a portfolio of surgical and nonsurgical treatments for the "destroyed elbow".
